    Análogo de mucosa oral basado en el co-cultivo de fibroblastos gingivales y queratinocitos

    Objetivo: la presente investigacion tiene por objetivo describir el desarrollo del co-cultivo celular in vitro de un analogo de encia. Materiales y metodos: A partir de pequenas biopsias de encia en ratas Sprague Dawley, se hicieron cultivos primarios de fibroblastos y queratinocitos, una vez alcanzada confluencia celular del 80% se procedio a sembrar los fibroblastos en la malla de colageno, dos semanas mas tarde se sembraron los queratinocitos. Resultados: Durante la sexta semana las celulas formaron monocapas con disposicion ordenada y se diferenciaba claramente la morfologia fusiforme, alargada y de nucleo redondeado en posicion central caracteristico de los fibroblastos gingivales. Los queratinocitos se observaron como celulas irregularmente poliedricas con nucleo central y citoplasma de aspecto hialino. Basado en los principios y tecnicas de ingenieria tisular y haciendo uso de una membrana de colageno bovino marca Mem-Lock® de la casa comercial BioHorizons®, fue posible obtener un co-cultivo celular utilizando dicha membrana como scaffold en un periodo de tiempo de 4 semanas de cultivo Conclusion: En esta investigacion fue posible aislar y cultivar exitosamente los dos principales tipos celulares que constituyen la mucosa bucal a partir de muestras tomadas de ratas Sprague Dawley.

    Análogo de tejido conectivo a partir de fibroblastos gingivales sembrados en membranas de colágeno comercial

    Modern dentistry using guided tissue regeneration and tissue engineering in periodontal defects and gingival recession treatment, applying surgical techniques such autologous grafts have shown excellent results. The objetive was to determine the efficiency of gingival connective tissue analogs, obtained from cultured fibroblasts seeded on a commercial collagen scaffold, for grafting in Wistar rats induced gingival recession. The methodology Connective tissue analog was developed sowing a human gingival fibroblasts culture on a collagen scaffold Membracel®-O, at the same time it was induced a 2 mm gingival recession in the central incisors of three Wistar rats, lifting a Newman flap to create a marginal bone dehiscence; 45 days after, the connective analog graft was inserted in rat 2, rat 3 received a Membracel ®-O graft without cells and rat 1 didn’t received any graft leaving it as a control, finally the animals were clinically evaluated every week before euthanasia. The results: Rat 1 maintained the 2mm gingival recession, rat 2 achieved an insertion gain of 2 mm also presenting increased biotype and rat 3 presented a 1.5 mm gingival recession. It can be suggested that using Membracel ®-O to produce connective tissue analogues may be an alternative to gingival recession treatment since insertion gain was achieved with no evidence of clinical signs of rejection.

    Análogo de tejido conectivo injertado en recesiones gingivales inducidas en ratas Wistar

    Mucogingival surgery aim is to prevent or treat anatomical, pathological and traumatic gingival defects, such as gingival recession, which cause functional aesthetic and biological difficulties. To treat them, it’s been developed, tissue substitutes obtained from gingival connective cells of the patient. The purpose of this study is to determine the effectiveness of an analogue connective tissue developed from human gingival fibroblasts, by the treatment of induced gingival recession. 24 male Wistar rats were selected, the periodontal clinical examination was carried out and gingival recession in right central upper incisor was induced; then after cicatrization it was made a flap to place the analog connective tissue graft obtained from a culture of human fibroblasts, in animals of group I, group II received a collagen scaffold without cells and group III were left without graft. The study results showed that 86.66% of the animals in group I had a 2mm insertion gain, while 13.3% won 1.5mm insertion. 86% of the animals of group II won 1mm insertion, and group III had not showed insertion gain, instead they remained with 2mm recession. In conclusion the gingival graft from a fibroblast cell culture can be considered as a viable alternative to achieve root coverage, and even increase the width of attached gingiva and thickness of periodontal biotype.

    Psoriasis is a chronic inflammatory mucocutaneous disease that affects 2% of the population. Cutaneous lesions appear as erythematous plaques localized or generalized papules covered with white scales hyperkeratotic, oral manifestations and irregular erythematous lesions with elevated white borders, ulcerations and desquamative gingivitis. It may be asymptomatic or cause pain, burning and sensation of roughness. Its etiology is unknown, although the interaction between the immune system and environmental factors may activate T cells skin, epidermal keratinocytes, with increased cytokine and tumor necrosis factor (TNF). The objective is to provide clinical evidence on the periodontal manifestations of patients with psoriasis, and reports a clinical case of a 62 year old male patient, who filed the anamnesis medical diagnosis of psoriasis, controlled systemically and presence of oral manifestations, was treated with mechanical therapy, periodontal and use of local antiseptics. At the end of 2 months of reassessment was a normal clinical characteristic of periodontal tissues. It was concluded that the oral lesions can present spontaneous remission or respond to systemic therapy, topical corticosteroids, mouthwash buccal or palliative therapy. Diagnosis and successful treatment that includes a proper control of dental plaque in patients with psoriasis allow control periodontal disease and reduce the appearance of severe injuries.

    Efectividad del Quitosano como coadyuvante en el tratamiento local de periodontitis crónica

    The present study, experimental, aimed to determine the effectiveness of chitosan as an adjunct in the treatment of chronic periodontitis. This research involved 12 patients diagnosed with chronic periodontitis who attended the periodontics clinic of the Faculty of Dentistry at the University of Los Andes. The sample was divided into experimental side, which was subjected to conventional therapy of scaling and root planing plus application of chitosan and hand control, which received only conventional therapy. The evaluation was performed by clinical parameters such as periodontal index, recorded before, during and after treatment, with controls for 6 weeks. Data were analyzed using descriptive statistics and statistical test was used Fisher's exact test, which revealed that the improvement of the disease, is significantly associated with the application of chitosan as an adjunct to scaling and root planing, as this biomaterial provides a quick and efficient recovery of the periodontal tissues.

    Eficacia del gel con extracto de las hojas de Solanum Lycopersicum como terapia complementaria para la gingivitis inducida por placa

    Gingivitis has been treated conventionally and with natural medicine. The leaves of the tomato plant have been used as an alternative periodontal treatment; therefore, the aim of this study was to determine the effectiveness of a gel prepared with the extract of the plant in the treatment of gingivitis induced by dental plaque. The study group was integrated by 30 patients diagnosed with gingivitis induced by dental plaque. The research was developed in two (2) stages: A pharmacology stage, in which was prepared de gel plus the extract of the leaves of tomato plants and a clinical stage, in which a clinical exam was carried out to each patient and treatments were applied to both, experimental and control groups. O´Leary, Gingival and Hemorrhagic indexes were measured before and after treatments application on both groups. The statistic analysis of the results was carried out, and for the three (3) indexes significant values were obtained, therefore, we conclude that the conventional treatment plus the use of the gel containing the extract of the tomato plant showed to be an effective therapy treatment to treat gingivitis induce by dental plaque.

    Enfermedad periodontal necrosante

    Acute periodontal diseases are rapid onset clinical conditions that involve the periodontium or associated structures and are characterized by pain or discomfort, tissue destruction and infection. In contrast to most other periodontal conditions, they can cause rapid destruction of periodontal tissues, thus emphasizing the importance of rapid diagnosis and treatment. Among these conditions, the following stand out necrotizing periodontal disease. According to the International Workshop for Classification of Periodontal Diseases and Conditions 2017 are two: Necrotizing gingivitis (NG) and Necrotizing periodontitis (NP). Necrotizing stomatitis (NE) is a more aggressive variation, which can be linked to the two entities mentioned. Necrotizing periodontal disease is an infection characterized by gingival necrosis, pain and gingival bleeding. It is considered to be an acute opportunistic gingival infection caused by dental biofilm. It appears more frequently in undernourished children and young with immunodeficiency. In its pathogenesis, there are factors related to the oral microbiota and factors associated with the host. Diagnosis is made by a clinical examination. The purpose of this study is to conduct a literature review of necrotizing periodontal disease. The clinical presentation, the course and the response to EPN therapy are specific among other periodontal diseases, so the existence of a particular clinical disease category is supported.

    Enucleación de un quiste periapical y utilización de plasma rico en plaquetas . Reporte de un caso

    El quiste periapical inflamatorio es una lesión odontógena frecuente en cavidad bucal, se\npuede presentar como pequeñas lesiones que involucran la zona periapical o grandes lesiones que\ncomprometen estructuras anatómicas y conllevan a pérdidas óseas considerables. Actualmente\nexisten innovaciones en cuanto a materiales utilizados para acelerar la regeneración ósea posterior\na la presencia de quistes periapicales en cavidad bucal. El plasma rico en plaquetas (PRP) es una\nexcelente opción al alcance de todos los cirujanos orales, por su facilidad de obtención del mismo\npaciente y capacidad de cicatrización de los tejidos. Este artículo reporta el caso de un paciente de\n29 años que acudió a la Facultad de Odontología de la Universidad de Los Andes con una lesión\ninflamatoria de gran tamaño en la región maxilar anterior, se establece un diagnóstico presuntivo\nde quiste periapical inflamatorio el cual fue tratado con enucleación y colocación de PRP en el\nlecho quirúrgico. Se realizó control radiográfico post operatorio a los siete días, al primer, tercero y\nsexto mes en el cual se observó una excelente cicatrización del tejido óseo; se concluyó que el PRP\nes una opción válida de tratamiento para los quistes periapicales.

    Evaluación de la eficacia de la técnica de cepillado de bass modificada a través de un metodo de enseñanza intraoral y otro extraoral

    Teaching the mechanical control of supragingival plaque is very important because its purpose is to prevent periodontal disease and create a positive attitude in the patient, inform and motivate. It aims to assess the effectiveness of the Bass brushing technique modified by teaching methods intraoral and extraoral, for mechanical control of dental plaque. We selected 40 patients between 20 and 30 years old, male and female, divided into two groups: group A (20 patients) who received a demonstration of the technique of brushing with intraoral method and group B (20 patients) who received a demonstration of the technique of brushing with extraoral method. Variable was controlled reinforcement and motivation brushing technique, dividing the groups A and B into four subgroups, groups (A1 and B1) were motivation and reinforcement, and the other two groups (A2 and B2) did not. The percentage of supragingival plaque before brushing technique explain between patients in group A was 53.47% while in group B was 45.98%. After explaining the brushing technique in group A decreased to 32.42%, while in group B decreased to 34.76% (p> 0.005). We conclude that the explanation of the technique of brushing with intraoral teaching method proves to be simple, practical and effective for the control of dental plaque,being even more efficient when combined with the reinforcement and motivation of the patient between appointments.

    Factores dentales y protésicos relacionados con la enfermedad periodontal

    This work aims to conduct a review of dental and prosthetic factors related to periodontal diseases. According to scientific evidence, the location of restoration margins within the junction epithelium and the insertion of supracrestal connective tissue may be associated with gingival inflammation and a potential recession. On the other hand, the anatomical factors of the tooth, root anomalies and fractures can act as retentive factors of dental biofilm that increase the probability of gingival or periodontal diseases. In conclusion, although the etiology of periodontal disease is mainly bacterial, the factors that promote the accumulation of dental biofilm or allow the entry of bacteria into the periodontium should be considered in the classification and diagnosis of periodontal diseases. Several factors related to tooth / root anatomy and restorative considerations have been associated with gingival inflammation, insertion loss and bone loss. However, the evidence especially related to the biological mechanisms by which these factors affect the periodontium are inconclusive. This highlights the need for additional studies and adequate periodontal evaluation.

    Lo que debemos saber sobre control de infección en el consultorio dental

    Each one of the members of the dental health professionals group, as well as the patient who goes to an appointment, are exposed to the risks of getting highly infectious diseases; in the last two decades, society has been more aware about the rising and transmission of infectious-contagious diseases. Patients and health workers have shown interest and concern about diseases transmitted by the Human Immunodeficiency Virus (HIV) and by hepatitis B and C virus (HBV and HCV), which has led the dental health team (dentists, dental hygienists) to star putting into practice biosecurity and infection control in the dentist office. Hence the importance of reviewing the latest concepts known to protect ourselves and the patient in dental practice following infection control regulations destined to diminish risk of infectious microorganisms from identified and unidentified sources in the dentist office linked to accidents by exposure to blood and bodily fluids.

    Rol del odontólogo en el diagnóstico precoz del pénfigo vulgar. Reporte de un caso

    El penfigo vulgar es una enfermedad mucocutanea que afecta usualmente a pacientes entre la cuarta y sexta decada de la vida, caracterizada por la aparicion de ampollas intraepidermicas que se originan por acantolisis y se debe a la produccion de autoanticuerpos frente a las glucoproteinas que forman los desmosomas. Antes de la terapia sistemica esteroidea, la enfermedad tenia un pronostico fatal para la mayoria de los pacientes. Hoy en dia ha mejorado el pronostico, gracias al diagnostico temprano y el tratamiento con corticoesteroides, antiinflamatorios e inmunosupresores. La cavidad oral casi siempre se encuentra afectada por esta patologia y las lesiones orales aparecen en la mayoria de los casos como primera manifestacion clinica, caracterizandose por la formacion de ampollas que se rompen rapidamente dejando areas alternadas de ulceracion y necrosis. El presente estudio tiene como finalidad resaltar el papel que juega el odontologo en el diagnostico precoz de la enfermedad, destacando que las lesiones descamativas, erosivas y ulcerosas pueden estar presentes en otras patologias mucocutaneas, por lo que es de vital importancia establecer el diagnostico definitivo basado en el estudio anatomopatologico.
