Rivero; Dina Gil; Pedro José SalinasespanolLa endometriosis perineal, se define como la presencia de tejido endometrial en la region perineal. La endometriosis perineal con afectacion del esfinter es una enfermedad poco frecuente. En muchos casos se puede confundir con patologias como granuloma, absceso, lipoma, adenopatia etc. El tratamiento ideal consiste en la excision amplia, pero en muchos casos se asocia con incontinencia fecal cuando existe afectacion del esfinter. Sin embargo, en la mayoria de los casos la excision puede ser incompleta, lo que amerita tratamiento adicional con terapia hormonal. Se describe el caso de una paciente de 45 anos con diagnostico de endometrioma anal y afectacion del musculo esfinter anal. Se procedio a realizar excision amplia de la lesion con preservacion del musculo esfinter anal y seguimiento durante un ano sin presentar complicaciones postoperatorias (incontinencia fecal). Se concluye que la endometriosis perineal constituye una forma infrecuente de presentacion de las endometriosis pelvicas. EnglishPerineal endometriosis is a disease characterized by the presence of endometrial tissue in the perineal region. Perineal endometriosis with anal sphincter involvement is an infrequent occurrence. In many cases it is confused with granuloma, abscess, lipoma, adenopathy. The best treatment to obtain satisfactory cure consists in the wide excision, but it may cause incontinence if the anal sphincter is involved. Conversely, narrow excision may result in incomplete removal, with increased recurrence rates and need of additional hormonal therapy. In this paper we describe a 45 year old patient, with diagnosis of perianal endometrioma with anal sphincter involvement. The patient went through surgery with wide excision and preserved anal sphincter. After one year follow-up there were not complications reported by fecal incontinence. The origin is due to diverse causes. It is a devastating disease with multiple results such as social isolation and reduced quality of life. It affects all ages. The normal continence of the stool is maintained by a complex mechanism of anatomy and anorectal physiology. Alterations in this mechanism causes fecal incontinence. There are several types of incontinence, classified by severity. There are multiple physiological studies complementing the diagnosis of the fecal incontinence. In its initial stages it can have positive response to pharmacological treatments. Most cases need surgical treatment with excellent results and improvement in the symptoms of the intestinal function
