Item type: Item , A novel IoT system for remote monitoring in geriatric rehabilitation(2025) Katerin Deysi Vasquez Benito; Eynar Calle Viles; Edgar Roberto Ramos Silvestre; Rommer Alex Ortega MartínezThis paper presents the design, development, and implementation of a geriatric rehabilitation kit consisting of four dynamic stations, aimed at improving the physical and cognitive well-being of older adults through the use of emerging technologies. The system employs specialized sensors, computer vision, interactive games, a web-based video game, and MQTT communication. It operates on an architecture of ESP32 development boards, sensors, and a VPS server for real-time data storage and analysis.Each geriatric station was designed to stimulate specific abilities: motor activity with a pedal board and video game, agility and memory through a cognitive panel, gait cycle rehabilitation with artificial vision, and balance improvement with the stairs and slope monitored by sensors.The results obtained from experimental tests conducted with geriatric patients at a senior care center demonstrated significant improvements in their physical and cognitive performance, as evidenced by the collected metrics and historical chart analysis. The combination of different sensors and their programming facilitates key data collection, optimizing physical therapy processes and promoting active aging. Furthermore, high acceptance was observed among patients and healthcare professionals, who highlighted the motivation the kit generates and the usefulness of remote monitoring. The pathogenesis is multifactorial; it depends on the factors that cause calcification, such as alteration of calcium metabolism, phosphorus, hyperparathyroidism, female sex, diabetes, obesity; from the clinical point of view, calciphylaxis can progress from a painful purpura to extensive skin necrosis, causing death from sepsis. Treatment is based on wound care and elimination of the factors. Thus, a clinical case is presented of a 64-year-old woman with a history of chronic kidney disease, diabetes, criteria for urgent dialysis, presence of skin lesions on the lower limbs, with suspicion of calciphylaxis, confirmed by anatomopathology, presenting an unfavorable evolution, with an increase in the size of the skin lesions, poor prognosis and mortality.Item type: Item , Caracterización de la plaquetopenia, leucopenia y aumento del hematocrito en la evolución y gravedad de los pacientes con diagnóstico de dengue(2021) Rommer Alex Ortega Martínez; Liliana Alejandra Cáceres SanchezObjetivo: caracterizar la presencia de la plaquetopenia, leucopenia y aumento del hematocrito con la evolución y gravedad de los pacientes con Dengue, en el Hospital Univalle, en la ciudad de Cochabamba, Bolivia. Material y métodos: estudio retrospectivo, longitudinal y analítico, desde el 2017 al 2020. Resultados: se identificaron 235 pacientes, 83% el 2020; el 65% entre 19 a 45 años; 54% de sexo masculino, 80% provienen de Cercado-Cochabamba, siendo más del 95% de la zona sud. La prueba de detección de la proteína NS1Ag (AccuBio Tech Co, Ltd.) se usó en 71%; 56% se hospitalizaron; más del 50% presentaron sintomatología entre el 2do y 5to día. Dentro la clasificación del dengue, el 75% perteneció a dengue sin signos de alarma (p=<0,01). La fiebre (p=0,001), asociada a la clínica con signos de alarma como el dolor abdominal, vomito persistente y sangrado (p=< 0,05) fueron frecuentes. Leucopenia < a 5 000/mm3 fue común en mujeres con signos de alarma, en contraposición con los varones; plaquetopenia en pacientes con signos de alarma y en varones; el hematocrito > 45% en varones, presentando más de 5 días de hospitalización (p=0,023). Conclusiones: plaquetopenia, leucopenia y el aumento del hematocrito son biomarcadores de severidad clínica y estancia hospitalaria, asociados a un diagnóstico precoz, empleando la sintomatología y pruebas rápidas disponibles; siendo necesario considerar la presencia de pacientes autóctonos de la zona sud de Cochabamba, Cercado.Item type: Item , Cerebral cryptococcosis regarding two clinical cases and bibliographic review(2024) E. Garcia; Rommer Alex Ortega MartínezCryptococcosis is a life-threatening infection, the species complexes Cryptococcus neoformans and Cryptococcus gattii are yeasts with a polysaccharide capsule, metabolize urea and catecholamines; It is transmitted by inhalation of bird droppings, mainly pigeons, and is capable of causing outbreaks in both immunocompromised and immunocompetent hosts. In Latin America, cryptococcal meningitis is a health threat. The initial infection is localized to the lungs and spreads to other organs, such as the central nervous system, where it causes meningoencephalitis and rarely focal granulomatous lesions such as cryptococcomas. The diagnosis must be made early, with serological tests for the cryptococcal polysaccharide capsular antigen. Treatment is divided into induction, consolidation and maintenance. Below, two clinical cases are presented, the first case is a 68-year-old female with a history of tuberculosis 15 years ago; 10 days, with holocranial headache, nausea and vomiting; on physical examination with superficial stupor, nuchal rigidity; kerning sign (+); lumbar puncture with identification of cryptococcal meningitis, induction treatment with fluconazole was started. The Epstein-Barr virus (EBV) is a double-stranded DNA gamma herpesvirus that presents a latent infection and lytic replication; it can diffuse into the central nervous system and alter the integrity of the blood-brain barrier, being associated with neurocognitive impairment, neuronal damage and inflammation. In relation to tuberculosis, it became the second infectious disease that caused the most deaths in the world after COVID-19; tuberculous meningitis is considered the most severe form of extrapulmonary tuberculosis (TB) with a mortality of 70% in low-income countries. Below is the case of a 35-year-old man with a history of adrenal insufficiency and hyperthyroidism, who was receiving corticosteroids; he went to the emergency service with a 5-month history of clinical symptoms characterized by holocranial headache, focal retrograde amnesia, periods of altered state of consciousness, dizziness, nausea that led to vomiting on several occasions, with sudden loss of consciousness. , accompanied by involuntary tonic-clonic movements and hearing loss; after the diagnostic screening, EBV and TB are identified; receives specific treatment with good clinical evolutionItem type: Item , Consequences of the academic evaluation stage on the habits and physical health of students in the medicine career of the University del Valle(2024) Carlos Javier Salgueiro Lizarazu; Rommer Alex Ortega MartínezIntroduction: Academic evaluation is a necessary stage in the training of the medical student; It can end in negative habits and stress that can influence physical health and performance; In this way, the objective of this work is to determine this relationship that directly affects medical students. Material and methods: Cross-sectional study in 110 medical students from the Universidad Privada del Valle in Cochabamba, between 18 and 25 years old; through closed surveys, their study habits and health were evaluated during evaluation stages; The information was processed in SPSS (Statistical Package for the Social Sciences) version 24 and Microsoft Office Excel, version 2021. Results: 73 % of students reported negative effects on their habits during exams. 51 % indicated sleep problems, 31 % nutritional neglect and 19,8 % an increase in tobacco consumption; additionally, 80 % experienced weight changes, and 70 % slept less than 6 hours. Discussion: Academic stress generates negative changes in student well-being, affecting sleep, eating and tobacco consumption; these problems are common in Latin America; it is recommended to implement stress management programs and promote healthy habits in the evaluation stage.Item type: Item , Design and Development of a Podiatric Station Prototype for the Diagnosis of Diabetic Foot(2025) J Jiménez; Rodrigo Martínez Severich; Eynar Calle Viles; Rommer Alex Ortega MartínezDiabetic foot is one of the most frequent complications of type 2 diabetes mellitus, characterized by the development of ulcers, infections, and a high risk of amputation. In Bolivia, approximately 6.5% of the adult population lives with this condition, and progressive foot injuries are common. In response to this problem, a functional prototype of a podiatric station—named PODIATECH—was designed and developed to support early diagnosis through structural and thermal foot analysis. The system comprises two main modules: a structural module that uses a flatbed scanner to capture the plantar footprint and applies the Hernández-Corvo index; and a thermal module that uses infrared imaging to detect temperature differences associated with circulatory alterations. The entire system is managed through a web-based platform that enables patient registration, clinical record consultation, and automated report generation. During preliminary validation, ten participants were evaluated, five of whom had a confirmed diagnosis of diabetes. In four of these cases, temperature differences greater than 2 °C were detected, corresponding to clinically relevant risk zones. No significant alterations were found in healthy participants. These results suggest that the system may serve as an effective tool for early diabetic foot screening. Further clinical trials with a larger sample size are recommended to assess the system’s performance and reliability in real-world settings.Item type: Item , Design and Implementation of a Didactic Monopolar Electrosurgical Simulator for Biomedical Engineering Education(2025) Nicolas Nava Angulo; Elías Chávez Jaldín; Rommer Alex Ortega Martínez; Eynar Calle VilesThis work presents the development of a didactic monopolar electrosurgical unit (ESU) simulator, designed as a support tool for the technical training in the handling and basic operation of this medical device within academic environments. The simulator aims to facilitate both theoretical understanding and practical experience of the electrosurgical unit’s operating principles, allowing students to interact with a functional system that replicates its primary modes: cutting and coagulation. The system architecture is based on an ESP32 microcontroller, which generates PWM signals within an adjustable range of 200 kHz to 1 MHz, controlled via rotary encoder potentiometers, and displayed in real time through an LCD screen with I2C interface. The power stage integrates an IRFZ44N MOSFET and a high-frequency transformer, configured to maintain energy levels within safe limits. The activation circuit incorporates 4N25 optocouplers, ensuring electrical isolation between user controls and the power electronics. This simulator is conceived as a formative platform capable of replicating realistic operational scenarios, promoting safe, scalable, and practical learning—particularly suited for biomedical engineering education and related disciplines.Item type: Item , Doppler transcraneal y su impacto en el pronóstico de la lesión cerebral traumática, Hospital Clínico Viedma, Cochabamba-Bolivia.(2023) Cristhian Huanca; Alejandra Natalia Espinoza Marquéz; Rommer Alex Ortega MartínezIntroducción: La lesión cerebral traumática (LCT) es una causa importante de muerte y discapacidad; predecir el resultado, es de utilidad en la toma de decisiones. El doppler transcraneal (DTC), es una herramienta diagnóstica y monitorización del flujo sanguineo cerebral; podría establecer un pronóstico confiable y temprano. El objetivo es evaluar sí las características del DTC es eficaz como indicador pronóstico. Material y métodos: Se realizó un estudio prospectivo de pacientes con LCT. Se efectuaron mediciones con DTC al ingreso y su estadía; el índice de pulsatilidad se calculó́ mediante la fórmula de velocidad sistólica y diastólica, dividida con la velocidad media (VS-VD/VM); se consideró hipertensión endocraneana si el índice de pulsatilidad (IP>1,2); el resultado se evaluó por el puntaje GOSE (Glasgow Outcome Scale Extended) a los 3 meses (información obtenida vía telefónica), dicotomizada: Favorable (4-5) y desfavorable (1-3). Resultados: Fueron incluidos 49 pacientes; el resultado desfavorable a los 3 meses fue del 63,2 %; el análisis del grupo con DTC anormal tiene la probabilidad de presentar 54 veces más de una evolución desfavorable (RR: 54; IC 95 % (8,8-32,8); p<0.0005); Sensibilidad 78 %; especificidad 81%; AUC 0,850, y menor probabilidad de sobrevida a los 14 días (p=0,021); variables independientes asociadas: Escala de coma Glasgow (ECG) cercano a 3 puntos RR: 33; IC 95 % (1,31-10,6); hipotensión RR: 9,28; IC 95 % (1,49-5,7); craniectomía RR: 1,96; IC 95 % (1,01-4,41); pupilas fijas RR: 3,4; IC 95 % (1,15-9,9); Escala de Marshall en conjunción con la ECG mayor sensibilidad y especificidad. Conclusión: Estos datos mostraron una posible eficacia del DTC como herramienta diagnóstica y además pronóstica en la lesión cerebral traumática.

Item type: Item , Embolia por colesterol en miembros inferiores: reporte de caso(2023) Maiko Alejandro Tavera Diaz; Annia Aguilar Loayza; Rommer Alex Ortega MartínezLa embolia por cristales de colesterol es efecto de la desestabilización de una placa de ateroma tras un evento desencadenante, produciendo la migración de cristales de colesterol hasta arteriolas periféricas, desencadenando un proceso inflamatorio endotelial; el espectro clínico varía desde ser asintomático hasta con un compromiso multiorgánico; la sospecha diagnóstica es principalmente clínica y será la biopsia de piel por su fácil accesibilidad, la que confirme el diagnóstico. El tratamiento es aún controvertido y no existe un consenso de las medidas terapéuticas para aplicar. A continuación, se presenta el caso de un paciente varón de 72 años de edad con una ateroembolia por cristales de colesterol en miembros inferiores, secundaria a una manipulación endovascular por angioplastia previa.Item type: Item , Evaluación de la depresión en niños y adolescentes oncológicos, Hospital del Niño Manuel Ascencio Villarroel de Cochabamba, Bolivia(2024) A.A Peredo; Haldrin Antonio Bejarano Forgueras; Rommer Alex Ortega MartínezEl cáncer y la depresión son enfermedades prevalentes, con una incidencia creciente en los últimos años; la asociación entre la depresión y el curso clínico de la enfermedad oncológica en niños y adolescentes es un desafío; ya que el hallazgo de esta enfermedad influye en el desempeño, abandono, fracaso y recaída al tratamiento. Métodos: estudio analítico y longitudinal, aplicado al momento del diagnóstico y a la edad actual en menores entre siete a diez y ocho años; se utilizó el índice de Beck para la valoración del estado de depresión y la escala de Graffar modificado para el nivel socioeconómico familiar. Resultados: cincuenta y cuatro pacientes fueron incluidos en el estudio; la mediana de edad actual fue de doce años, más del 39 % provienen de Cercado; la leucemia linfoblástica se presentó en un 57 %; el estadio de la enfermedad está en relación con el nivel de depresión, que es moderada en la recaída y mínima en la etapa de vigilancia; se identificó una relación entre el nivel de depresión con la percepción de la depresión, el estadio de la enfermedad y el empleo de la terapia quirúrgica (p<0,05); mediante un análisis para las 3 variables, se encuentra relacionadas con el nivel de depresión y es principalmente moderado (V de Cramer: 0,6). Conclusión: los pacientes presentan depresión en grados leve a moderado, destacando una relación causal entre la ansiedad por separación y el grado de depresión, así mismo con el procedimiento quirúrgico y el estadio de la enfermedad.Item type: Item , Family satisfaction in the Intensive Care Unit: A multidimensional network analysis approach(2025) Rommer Alex Ortega Martínez; Roly Misael Ramos Zenteno; Carmen Laura Garcés Hazou; Alejandro Pardo Ledezma; Natalia Andrea Cuadros Pariente; Niciel Poma CruzIntroduction: Family satisfaction in the Intensive Care Unit (ICU) is a multidimensional construct for the quality of care. Understanding its structure and interrelationships requires advanced statistical analysis. The aim of this study was to explore the latent structure of family satisfaction, evaluate the instrument's reliability, and analyze its relationships. Materials and methods: A cross-sectional study was conducted with 47 patients' relatives. Cronbach's α, exploratory and confirmatory factor analysis, network analysis, latent class analysis, and a Bayesian approach were applied. Finally, mediation and moderation analyses were performed. Results: The instrument demonstrated excellent reliability (α = 0.954). Factor analysis confirmed a four-factor structure that explained 77.8% of the total variance. Network analysis identified physician skill and honest information as the most central nodes. Latent class analysis revealed three distinct profiles: highly satisfied, moderately satisfied, and dissatisfied. Bayesian analysis provided evidence that physician skill and honest communication are predictors of satisfaction. Mediation and moderation analyses showed that medical communication mediates the relationship between professional competence and satisfaction, with stronger effects in women and family members living with the patient. Discussion: The application of advanced statistical analysis allowed a comprehensive understanding of family satisfaction in the ICU, confirming its multidimensional structure. The skill of physicians, nurses, and transparent communication are the pillars of satisfaction.Item type: Item , Gangrena de Fournier y miocardiopatía séptica a propósito de un caso clínico: un diagnóstico infravalorado(2024) Rommer Alex Ortega Martínez; Pachakutek Illescas Gonzales; Bismarck Raúl Ibañez VelascoFournier's gangrene, frst described in 1883, is a necrotizing infection that occurs in the perineal and abdominal areas. It develops in patients with risk factors such as diabetes mellitus, obesity, alcoholism, hypertension, atherosclerosis, and immunosuppression. Its etiology is a polymicrobial synergy of gram-positive, gram-negative, and other pathogens, which require the presence of an initial lesion in the genital or perineal region and are responsible for tissue destruction, endarteritis, and microthrombosis. Systemically, they cause multiorgan failure, such as myocardial dysfunction induced by sepsis or septic cardiomyopathy, frst described in 1984, which can manifest as systolic or diastolic dysfunction of both ventricles, presence of cardiac arrhythmias, preceded by QT interval prolongation, and elevated cardiac troponins. This is similar to the presentation of our 71-year-old patient who had a favorable outcome.Item type: Item , ¿Guillain Barre syndrome and inadequate secretion of antidiuretic hormone, is their relationship possible?(2024) Carmen Laura Garcés Hazou; Rommer Alex Ortega Martínez; Giovana Marleny Sanabria GarcíaGuillain-Barré syndrome (GBS) is a polyradiculoneuropathy of acute onset and the most common cause of acute flaccid paralysis. It is characterized by the appearance of sensory and/or motor symptoms, accompanied by a decrease or absence of muscle stretch reflexes and other symptoms such as facial paralysis, altered eye movements, bulbar symptoms, altered wakefulness, dysautonomia and ataxia. On the other hand, hydroelectrolyte disorders are associated with serious neurological diseases as a poor prognostic factor; the most affected electrolyte is sodium; in fact, hyponatremia is the most common disorder in hospitalized patients, regardless of the cause; in GBS, it is a frequent finding on admission. It is important to recognize hyponatremia, due to its high morbidity and mortality; it is also a marker of an underlying disease. Syndrome of inappropriate secretion of antidiuretic hormone (SSIADH) is the most common cause of hyponatremia and has been associated with Guillain Barre syndrome; in this way, its diagnosis is a challenge for the doctor; finally, below we describe details of a 70-year-old male patient who was admitted to the intensive care unit, with hyponatremia with a SSIHAD, with good subsequent evolution.Item type: Item , Ictus Isquémico en Paciente Joven con Etiología Cardioembólica: Reporte de Caso Clínico y Revisión Contextual(2025) Walter Yuri Pérez Olazabal; Aracely Zapata Rojas; Waldo Marcelo Olmos Bayá; Rommer Alex Ortega MartínezIschemic stroke in young patients, particularly women without comorbidities, is an uncommon but increasingly relevant clinical presentation. We report the case of an 18-year-old female patient in Cochabamba, Bolivia, with no prior medical history, who presented with right-sided hemiparesis, aphasia, and sudden loss of consciousness. Brain CT revealed a large hypodense area in the left occipito-parietal region (75x50x40 mm), consistent with ischemic edema without hemorrhage or midline shift. Brain MRI showed focal hyperintensity in the left MCA territory on DWI, indicating restricted diffusion; T2 images revealed diffuse hypointensity and loss of cortico-subcortical differentiation, compatible with subacute ischemic injury. Angio-MRI demonstrated focal hypoperfusion and reduced arterial caliber. Transesophageal echocardiography identified mitral valve vegetation and intracavitary thrombi, suggesting a cardioembolic etiology. Despite intensive ICU management, the clinical course was unfavorable, with progressive hypoxic encephalopathy. Wake EEG showed persistent bilateral flat tracing with no cortical activity or stimulus response, confirming brain death. This case illustrates a fulminant form of juvenile stroke, emphasizing the importance of early comprehensive evaluation with advanced neuroimaging and coordinated multidisciplinary care.Item type: Item , Identificación del Aedes aegypti en la zona sud de Cochabamba, Bolivia(2024) Rommer Alex Ortega Martínez; Jorge Carlos Ruiz De la Quintana; Gloria BlancoObjective. To determine the presence of Aedes aegypti in the southern region of Cochabamba(Bolivia) to establish an updated record. Methods. A descriptive, prospective, and ecological study was conducted in three districts (6, 8, and 9) of the southern region of the Cochabamba Department, Bolivia, located above 2500 meters above sea level. Ovitraps were used for vector identification and subsequent egg hatching. Results. One larva of Ae. aegypti and one of Culex were identifed in district 6. In district 8, seven Ae. aegypti larvae and nine adults were found, along with two pupae and fve eggs in different Territorial Base Organizations (OTBs). Additionally, 56 Culex eggs were identifed in districts 8 and 9, 93 Culex larvae in district 8, and more than 40 Culex adults in the same district. The dendrogram revealed two clusters: one comprising all life stages of both species and another containing only Culex eggs. Conclusions. It is essential to implement vector elimination programs to prevent further infections caused by this arbovirus, particularly Ae. aegypti.Item type: Item , Invasive Klebsiella syndrome in a case in Bolivia(2025) Carmen Laura Garcés Hazou; Rommer Alex Ortega Martínez; Alejandro Pardo Ledezma; Natalia Andrea Cuadros ParienteKlebsiella pneumoniae is a Gram-negative bacterium that requires oxygen and is capable of fermenting lactose. It can cause serious infections such as pneumonia, bacteremia, and liver abscesses. There are two main subtypes: the classic strain (cKP) and the hypervirulent strain (hvKP), the latter being identified by its highly viscous mucus phenotype and its remarkable ability to invade.The most common hvKP lineage is ST23-K1, although K2 lineages (ST65, ST86, ST66) have also been recognized in different parts of the world, including South America. hvKP can affect healthy individuals and those with additional diseases, with diabetes mellitus being a relevant risk factor due to its association with immune dysfunction and hyperglycemia, conditions that favor bacterial growth and spread through the circulatory system. The case presented concerns a 45-year-old diabetic woman who was admitted with severe diabetic ketoacidosis and pneumonia caused by Klebsiella pneumoniae. Coordinador del Departamento de Investigación Clínica.Objective: To design and implement a comprehensive platform for the real-time tracking, traceability, and monitoring of infusion pumps, incorporating a prototype of a peristaltic infusion pump, with the purpose of optimizing intravenous therapy monitoring and enhancing the efficiency of medical and nursing staff. Materials and methods: A prototype of a peristaltic infusion pump was developed using 3D printing, a method that enables the precise and customized creation of highly durable and functional components. The prototype integrates microcontrollers and sensors capable of detectinganomalies during drug administration, ensuring constant flow control. In parallel, a digital platformwas designed with access through a web interface, allowing continuous real-time monitoring ofthe pumps. This low-cost solution, developed in Bolivia, aims to facilitate implementation insettings where existing commercial systems—although technologically advanced—are less accessible due to their high cost. The proposed system centralizes supervision and optimizes medical staff resources. Results: A series of experimental tests were conducted to evaluate the prototype’s accuracy compared to standard commercial infusion pumps by analyzing the percentage error in infusion rates. The prototype achieved an acceptable error range between -2.68 % and +2.57 %, demonstrating its accuracy and functionality. Furthermore, the digital platform proved effective in optimizing monitoring time, enabling healthcare personnel to focus on higher-priority tasks. Conclusions: The implementation of this comprehensive platformand the first prototype of a peristaltic infusion pump marks a significant step forward in intravenous therapy monitoring in Bolivia. Its low-cost design makes it a viable alternative for expanding access to real-time monitoring, promoting technological innovation, and optimizing both human and material resources in hospital settings.
