Nrf2 aqueous humor shows a higher concentration when GFAP is low in patients with diabetic cataracts versus those with senile cataracts
(2025) Selma Alin Somilleda-Ventura; Astrid Lucero Espinosa Soto; L Cerrillo; Otto Berger; Rosario Gulías-Cañizo
Oxidative stress is involved in aging and ocular diseases, so it is necessary to evaluate the serum and aqueous humor concentrations of Nrf2 and GFAP as possible indicators of neuro-oxidative damage in patients with senile or diabetic cataracts. Observational, prospective, cross-sectional, and comparative study; we obtain serum and aqueous humor samples from patients with senile cataracts (reference group) or diabetic cataracts. Concentrations of Nrf2 and GFAP were determined by the ELISA technique, compared with the Student´s t-test, and correlated with Spearman´s Rho test. Eighty patients included, 36 with senile and 44 with diabetic cataracts. The glucose concentration was 40.75 mg/dL higher in the diabetic group (p < 0.001), as well as the proportion of systemic arterial hypertension (p = 0.02). Concentrations of Nrf2 and GFAP were only quantifiable in the aqueous humor samples, showing opposed values between the groups (Nrf2 was significantly superior in patients with diabetic cataracts, and GFAP higher in patients with age-related cataracts, without statistical difference); positive correlations were higher in when diabetic cataracts exist (r = 0.59). Our findings suggest an accentuated response of the Nrf2 pathway by the significant increase of this molecule in patients with diabetes and cataracts possibly to compensate for the ROS overexpression, meanwhile in patients with senile cataracts, GFAP has higher (but non-statistical) concentrations, maybe due to a lower Nrf2 modulation.

Abstract Purpose : The relationship of Helicobacter pylori infection with ocular diseases, including anterior uveitis, has been reported. The objective of this study was to determine the presence of anti-H. pylori IgG antibodies in patients with idiopathic non-granulomatous anterior uveitis and compare the results with a control group. Methods : A prospective, comparative and cross-sectional study was conducted. Patients with idiopathic granulomatous anterior uveitis and a group of control subjects were included. The presence of anti-H. pylori IgG antibodies was determined. Chi square test was performed for comparative analysis with GraphPad Prism V5.0 software Results : 30 patients with idiopathic non-granulomatous anterior uveitis and 35 control subjects were included. In the determination of anti-H. pylori IgG antibodies, 24 (80%) patients and 19 (54%) control subjects were positive. A significant difference (p = 0.0263) was found between the groups and a odds ratio (OR) of 3.37. Conclusions : A direct relationship was found between the presence of anti H. pylori IgG antibodies and idiopathic non-granulomatous anterior uveitis. An association can be established between idiopathic non-granulomatous anterior uveitis and H. pylori infection, without this being a causal or physiopathogenic relationship.

Abstract Background: Visual-related quality of life is not always included as a variable of surgical success. However, some questionnaires such as the Visual Function Quality-25 in the Spanish version have proven their reliability and validity, but the quality of life in patients undergoing surgery has not been explored in the Mexican population; so, this study aims to identify it in patients with the most prevalent vitreoretinal diseases. Methods: The Visual Function Quality-25 questionnaire was applied to 76 patients with advanced diabetic retinopathy, rhegmatogenous retinal detachment, and other causes of vitrectomy, pre-and postoperative. It was divided into 10-domains and interpreted according to the National Eye Institute scores, where the highest value was the best visual function. Student's t-test for related samples and Wilcoxon's t-test were used to compare each domain between measurements, and Pearson´s R test to correlate age and quality of life total score; a p-value <0.05 was considered significant. Results: Diabetic retinopathy patients showed an improvement after surgery in all domains, one and three months after surgery; the higher scores were in those who used air o gas as tamponade. In patients with rhegmatogenous retinal detachment, there was an improvement in almost all the items from the first month, with a higher difference observed up to three months, while a decrease in ocular pain was observed in other causes of vitrectomy. Differences found in all the quality-of-life scores at one versus three months were not statistical, but clinically significant. Conclusions: The study shows that visual-related quality of life domains improve after vitrectomy, predominately in diabetic retinopathy subjects; inclusion of this analysis might be considered relevant within the parameters of surgical success of the most prevalent vitreoretinal diseases.
