    Clasificaciones Winter y Pell-Gregory predictoras del trismo postexodoncia de terceros molares inferiores incluidos

    Winter and Pell-Gregory’s classifications allow locating and evaluating radiographically the lower third molar. Trismus is an immediate post-surgery complication after extraction. The purpose of this study was to determine the predictive value from these classifications, in relation to the post-surgery of the above mentioned molars, in patients attended at the faculty of Dentistry of the Los Andes University in Merida, Venezuela. A randomized cohort study was performed with 81 patients between 17 and 33 years old and 127 lower third molar extracted were included in the study. The procedure initiated with a clinical and radiographic (panoramic) evaluation, molars were classified according to Winter and PellGregory’s classification including an eruption degree determination. The procedure was done under truncal  and mandibular infiltration anesthesia, the Winter modified and surrounded flap was designed and raised according indication with  subsequent exodontias of 38, 48 or both; the effective surgery time was registered and the primary closing was effected. The retirement of suture was done in 7 days and trismus was diagnosed. Then Chi-squared were significant for trismus with Winter, Pell-Gregory, gingival and bone retention and time of effective surgery. On having applied logistic multiple regression, it had significance for trismus, with Winter (p=0,040) and Pell-Gregory according to bone deepness (p=0,021). The predictive model was estimated for trismus in mesio-angular, vertical, horizontal, disto-angular inclusions (Winter) and A, B, C (Pell-Gregory) positions, and its value was obtained. We concluded that these classifications are predictors for trismus in post-surgery of lower third molar included.

    Condiciones de higiene bucal en niños en edad escolar

    La caries dental es una enfermedad donde intervienen factores internos: huesped, saliva y dieta y factores externos: condicion socioeconomica, habitos alimenticios, higiene bucal, nivel de educacion de la madre y habitos. Existe suficiente evidencia que la presencia de placa bacteriana es uno de los factores que contribuyen a la caries dental y que su remocion previene la aparicion de la misma. El presente estudio tuvo como objetivo evaluar las condiciones de higiene bucal en ninos en edad escolar, en la Escuela Basica “Filomena Davila” del Estado Merida, Venezuela. Se realizo un estudio descriptivo, transversal. La poblacion estuvo constituida por un total de 128 ninos, que cumplieron los criterios de inclusion, entre 6 y 13 anos de edad. Se aplico el Indice de Higiene Oral Simplificado (OHI-S). Se encontro que el 91,4 % de la poblacion estudiada presento detritus blando con predominio en los ninos de 8 y 11 anos, en contraste con el 8,6% que no presento. Al relacionar las variables presencia de detritus, genero y edad, se observa que los ninos de genero masculino presentan mayor cantidad y de esa poblacion, la edad mas frecuente es de 11 y 9 anos. El 79,7% no presento calculo dental, en contraste con un 20,3% que si presento. Al relacionarlo con genero y edad, se observa que la mayor cantidad fue encontrada en las ninas de 10 anos. Se concluye que el mayor porcentaje de la poblacion estudiada presenta un alto porcentaje de detritus blando con un menor porcentaje que presenta calculo.

    Estado de salud bucal en niños de instituciones educativas públicas y privadas del Municipio Libertador, Mérida-Venezuela.

    The determine the oral health status of school children and their relationship to the type of school: public and private. A correlational descriptive study was carried out in a population of 287 children, aged between 6 and 12 years, 143 children of La Salle School (private) and 144 children of La Salle Hermano Luis (public). We studied the presence of detritus, dental calculus and periodontal signs with descriptive analysis and Chi square. In the private school 95.8% presented detritus, 25.8% presented calculus and 34.9% presented signs of periodontal disease. In the public school 97.2% presented detritus, 32.6% presented calculus and 46.6% presented signs of periodontal disease. Was observed presence of detritus in 2/3 and 3/3 of the tooth surface in a high percentage of the public school children, while in the majority of private school children was observed in only 1/3 of the surface. The students of the public institution had higher rates of detritus (p = 0.0000) and of periodontal disease (p = 0.045) compared to private school children. No differences were found in relation to dental calculus. Using this indicator in larger studies, could quickly visualize the oral health status of Venezuelan children, to act accordingly in reducing the prevailing conditions, which may require not only curative actions but must be complemented by educational programs aimed specifically at children of public schools and their families, according to results reported by this study.

    La pasta iodoformada como tratamiento del absceso dentoalveolar crónico reagudizado. Caso clínico

    The purpose was to evaluate the clinical and radiographic behavior of a treatment of pulpectomia performed in molar primary with exacerbation chronic abscess dentoalveolar using iodoform paste as shutter material. For wich the pulpectomia was performed in primary molar with pulpar necrosis diagnosis and then to use iodoform paste as shutter material of the canal root. The molar was observed into 1, 6 months and 1, 2,3, 4 years after the treatment in stead to evaluate the apparition of clinic and radiographic sings and symptoms of pulpal pathology. A total remission was observed to exacerbation chronic abscess dentoalveolar, also, were neither clinical nor radiographic findings during the time of evaluation. Therefor , the iodoform paste is a material that can be used in the therapy to pulpar of molares primary with exacerbations chronic abscesses dentoalveolares. Its use as election treatment is suggested.

    Parestesia del nervio lingual post-exodoncia del tercer molar inferior incluido. Reporte de un caso

    The impacted lower third molar extraction can cause transitory injuries of the lingual nerve (between 2 % and 10 %) or permanent (below 0, 5 %). The aim is to present a case of lingual paresthesia, post-exodontia of the impacted lower third molar. We report 22-year-old female patient, who attended to the Development Mouth (bucal) Surgery Course from the Faculty of Dentistry of the ULA, with exodontia indication of the impacted lower third molar. After the clinical and radiographic evaluation, infiltrating and troncular jaw anaesthesia was applied. The modified Winter design and raising was performed with later 48 and primary closing extraction. The post-operatory control of suture removal was achieved after 7 days. The patient referred crawling sensation in the third and lateral edge of the right side of the tongue. A lingual paresthesia was diagnosed by the pinprick test. Weekly controls took place during 30 days. In conclusion, the modified flap design, the anaesthesia technique, the surgical time, the professional experience and the untimely maneuvers during the procedure are predisposing factors of post-surgical neuropathy. The recovery observed after four weeks confirmed the transitory affection of the lingual nerve.

    Prevalencia de la fluorosis dental, opacidad e hipoplasia del esmalte en niños en edad escolar del municipio Libertador del estado Mérida, Venezuela

    Objective: To determine the dental fluorosis frequency, enamel's opacity and hypoplasia in school kids from six to twelve years old of primary schools, bolivarianas' schools and educationals units of the Antonio Spinetti Dini parish, Libertador municipality of Merida state, Venezuela. Methodology: A descriptive essay realized of transversal design. The sample was compounded of 340 kids between six and twelve year old. The studied variables were dental fluorosis, enamel's opacity and hypoplasia, using dean's index and the enamel defect index (EDI) modified, according to health world organization (OMS) methodology. Results: 88% of studied population didn't present dental fluorosis, but it 12% of them presented, with prevalence of uncertain and very mild fluorosis (4.4% and 4.4% for each one). 84.7% of population didn't present defects with the enamel's development, in contrast to 15.3% who presented a prevalence of delimited and diffuse opacities (6.2% and 5.8% each one). Conclusions: The highest percentage of participants did not show either dental fluorosis or alterations in enamel development. Just a small percentage of children presented doubtful and very mild fluorosis as well as both delimited and diffuse opacities.

    Prevalencia de maloclusiones en niños del jardín de infancia de la Escuela Bolivariana “Juan Ruiz Fajardo”

    The aim of this study was to determine the prevalence of dental malocclusion in preschool children from Kindergarten to the Bolivarian School “Juan Ruiz Fajardo of Merida, Venezuela. We performed a cross-sectional study whose population consisted of 120 children between 3 and 6 years of age , of both genders. Clinical examination was performed with the child seated facing the examiner, using natural light and tongue depressors. The data obtained in the oral examination were recorded on Form Oral Health and degree of malocclusion was recorded according to the methodology described by the World Health Organization. It was observed that 54.9% of the study population had malocclusion which 47.1 % had moderate or severe malocclusion and 7.8 % slight malocclusion . It is concluded that due to a high percentage of dental malocclusion poblnacion has examined it is necessary to plan and execute preventive orthodontic treatment in order to achieve normal development of occlusion.

    Prevalencia y experiencia de caries en niños en edad escolar

    Se determino el estado de la denticion y necesidades de tratamiento en ninos en edad escolar, de la Escuela Bolivariana “Juan Ruiz Fajardo”, del Estado Merida, Venezuela, realizando un estudio descriptivo transversal, en una poblacion de 445 ninos; con edades entre 5 y 14 anos. Se aplico el Indice de estado de la denticion y necesidades de tratamiento segun metodologia descrita por la Organizacion Mundial de la Salud (O.M.S.). Se analizaron los datos mediante el paquete estadistico SPSS version 15.0. El 77,63% del total de la poblacion estudiada presenta caries; la experiencia de caries dental en la denticion temporaria fue de 53,91% y en la denticion permanente de 51,45%. En el estado de la denticion temporaria arrojo un ceo de 1,7. Y en la denticion permanente se determino un CPOD de 1,4. En cuanto a la necesidad de tratamiento se encontro que del total de la poblacion estudiada, en ambas denticiones requieren de tratamientos preventivos (70,04%). Considerando los indices CPOD y ceo se establecio que la prevalencia de caries dental en la denticion temporaria y permanente fue media, pero el estado real de la infeccion en la poblacion estudiada fue moderada destacandose la necesidad de atencion preventiva y la restauracion de la denticion afectada en esta comunidad.

    Prevalencia, experiencia y necesidades de tratamiento de caries de la infancia temprana en niños con labio y paladar hendido

    Cleft lip and palate is a craniofacial malformation that produces aesthetic, phonetic, hearing and dental problems to the affected child. The purpose of this research was to determine the experience, caries prevalence and treatment needs in children with cleft lip and palate at the Clinic for Cleft Lip and Palate of the Dentistry School, University of Los Andes in Merida, Venezuela.A descriptive cross-sectional study was performed. Sample was constituted by 97 childrenaged from 1 to 5 years old.Dentition Status and dental treatment needs indexes wereused, according to the methodology described by the World Health Organization (WHO). Data was analyzed using the SSPS statistical package, version 15.0.Results showed that 18.55% were affected by caries whereas 81.44% were caries free; upper right central incisor was the most affected tooth (50%), followed by the upper right first deciduous molar and the upper left central incisor (44.44%).It was observed aceod of 0.6. Treatment needs for children found were: preventive treatment (68.33%), one surface restoration (3,40%), two or more surfaces restoration (0,13%), pulpar therapy with restoration (0.06%) and extractions (0.33%).The prevalence of dental caries was very low and the higher percentage of required treatments is preventive.Presence of caries indeciduous first molarsshows a risk population for presenting caries in permanent teeth, which implies preventive dentist attention.
