Introduction: Third molar surgery is one of the most frequent dental practices, it is also considered an invasive surgical procedure that involves extensive tissue trauma and considerable postoperative inflammation. Dexamethasone is used to control postoperative sequelae due to the high effectiveness of its anti-inflammatory properties. An updated review is required that covers a greater number of routes of administration, for this reason, this study aims to determine the effectiveness of dexamethasone in the postoperative period of third molar surgery through three routes of administration based on a Systematic review of the literature. Method: The electronic search was carried out through information sources: Medline (via PubMed), Cochrane Library, Virtual Health Library (VHL), SciELO, LILACS, Link Springer, ELSEVIER (through ScienceDirect) and in the search engine Academic Google Scholar. The types of studies included were clinical trials, systematic reviews and meta-analyzes. Results: 63 studies were identified that met the inclusion criteria in which submucosal injection was shown to be the most frequently implemented route, compared to the other intramuscular and oral administration routes, however, all can be used in dental practice. with favorable results in the postoperative period of this surgery. Conclusions: According to the scientific evidence analyzed, the application of dexamethasone in third molar surgery is recommended in a single dose between 4-8 mg, to reduce pain, edema, swelling and trismus considerably.
