    Calidad del sueño en estudiantes de medicina durante la pandemia por COVID-19. Sleep quality in medicine students during the COVID-19 pandemic

    Objective: evaluate the quality of sleep in Venezuelan medicine students during May and June 2020, contingency due to the COVID-19 pandemic. Method: quantitative approach, evaluative type, cross-sectional design, not experimental. The sample consisted of 437 medicine students over 18 years of age from the universities of Venezuela during the COVID-19 quarantine, who gave their informed consent and responded to the survey prepared in Google Forms since May 1, 2020 until June 1, 2020. Results: 104 men (24.9%) and 313 women (75.1%), the ages were between 18 and 30 years, the mean and standard deviation was 22.61 ± 2,864 years. As for sleep time before quarantine, it was 6.844 ± 1.5776 hours, while during quarantine it was 7.936 ± 1.6757 hours, finding statistically significant differences (p <0.001). In the quality of sleep, differences were found according to sex (p = 0.002), specifically in the dimensions of insomnia by sex (p = 0.036), hyperinsomnia by sex (p = 0.016), hyperinsomnia by university (p = 0.003). Conclusions: The hours of sleep before and during quarantine showed differences, being greater after confinement. Regarding the quality of sleep, women were more affected in relation to insomnia and hyperinsomnia.

    Conocimiento, actitudes y prácticas sobre lactancia materna y COVID-19, en personal de atención pediátrica Knowledge, attitudes and practices on breastfeeding and COVID-19, in pediatric care personnel

    Objective: Establish the level of knowledge, attitudes and practices of pediatric health care personnel about breastfeeding and COVID-19. Methods: Analytical observational study, where a total of 219 subjects related to pediatric care were surveyed, in various locations, through the administration of an online survey. Demographic and academic aspects were taken into account, with emphasis on years of professional experience and the source of information. Results: An association was found between the level of knowledge about breastfeeding and COVID-19, classified as ‘Low’ and ‘High’, with respect to the years of professional experience of the pediatric care personnel surveyed (p= 0.021); 59,4% (n= 130) of the pediatric care personnel surveyed had a low level of knowledge, even when they presented a level of attitude and adequate practices. Conclusions: Most of the professionals surveyed do not have adequate knowledge in relation to breastfeeding and COVID-19, so it is required that the pediatric health team consider knowing and training with the scientific evidence that promotes or protects breastfeeding, in order to carry out evidence-based clinical practices that focus on the care of the mother, the newborn and his family.

    COVID-19 y lactancia materna COVID-19 and breastfeeding

    El trabajo tuvo como objetivo el estudio de publicaciones referentes al COVID-19 y lactancia materna basados en la evidencia disponible, asi como conocer las recomendaciones establecidas en cada circunstancia con la finalidad de optimizar la atencion de la madre y el nino. La revision se realizo en bases de datos PubMed, Sociedad Espanola de Neonatologia (SENEO), Sociedad Venezolana de Puericultura y Pediatria (SVPP), UNICEF, Organizacion Panamericana de la Salud (OPS) y Organizacion Mundial de la Salud (OMS), entre otros. Las investigaciones realizadas son muy escasas y estan enmarcadas en la proteccion y apoyo de la lactancia materna, que son considerados por las organizaciones y autoridades sanitarias de todo el mundo como un area de accion y atencion prioritaria. Hasta ahora no se ha identificado el virus en leche humana, aunque los estudios son muy limitados y debido a sus propiedades inmunologicas tiene mas probablemente un papel protector contra la infeccion que como vehiculo de transmision. Se concluyo que los resultados confirman la actualidad y pertinencia del tema para la proteccion de la salud de la madre y el nino lactante y la necesidad de realizar nuevas investigaciones relacionadas con la tematica de estudio. The objective of this work was to study publications related to COVID-19 and breastfeeding based on the available evidence, as well as to know the recommendations established in each circumstance in order to optimize the care of the mother and the child. The review was performed in PubMed databases, the Spanish Neonatal Society (SENEO), the Venezuelan Society for Childcare and Pediatrics (SVPP), UNICEF, the Pan American Health Organization (PAHO) and the World Health Organization (WHO), among others. The investigations carried out are very scarce and are framed in the protection and support of breastfeeding, which are considered by health organizations and authorities around the world as a priority area of action and attention. Until now, the virus has not been identified in human milk, although studies are very limited and due to its immunological properties, it has a more likely protective role against infection than as a vehicle of transmission. It was concluded that the results confirm the topicality of the theme for the protection of the health of the mother and the nursing child in and the need to carry out new research related to the subject of study.

    Estado nutricional en pacientes críticos pediátricos bajo ventilación mecánica Nutritional status in critical pediatric patients under mechanical ventilation

    La desnutricion en pacientes pediatricos hospitalizados continua siendo un hallazgo comun y una causa frecuente de aumento de la morbimortalidad en ninos y adolescentes, por ello se planteo como objetivo: analizar el estado nutricional en pacientes criticos pediatricos bajo ventilacion mecanica. Metodologia: estudio con enfoque cuantitativo, tipo analitico, diseno transversal de campo. Se incluyeron en la muestra a 37 pacientes pediatricos que ingresaron a la Unidad de Cuidados Especiales e Intensivos Pediatricos (UCEP/UCIP) del Instituto Autonomo Hospital Universitario de Los Andes (IAHULA) que se encontraban bajo ventilacion mecanica durante el periodo comprendido de enero a mayo del 2018. Se evaluo el estado nutricional al ingreso de la unidad por metodos directos e indirectos (antropometria, dietetica, evaluacion socioeconomica) y se compararon con la evolucion clinica de los pacientes. Resultados: 56,8% de los pacientes eran de sexo masculino, 45,9% lactantes menores, 51,4% se ubicaron en el nivel socioeconomico IV de la Escala de Graffar, el lugar de ingreso fue 70,3% UCEP y 29,7% UCIP, el diagnostico antropometrico se distribuyo en 2,7% sobre la norma, 51,4% en la norma y 45,9% bajo la norma, se encontraron diferencias estadisticamente significativas (p=0,040) entre evolucion del cuadro clinico y diagnostico antropometrico. Conclusion: El estado nutricional es un aspecto determinante para que los pacientes bajo ventilacion mecanica que ingresen en la UCIP/UCEP del IAHULA, evolucionen de forma satisfactoria o negativa de acuerdo al cuadro clinico de base. Recomendaciones: Evaluar nutricionalmente y mantener vigilancia epidemiologica nutricional constante a todos los pacientes que ingresen en la UCIP/UCEP del IAHULA. Malnutrition in pediatric hospitalized patients continues to be a common finding and a frequent cause of increased morbidity and mortality in children and adolescents, therefore the objective was set: to analyze the nutritional status of critically ill pediatric patients under mechanical ventilation. Methodology: study with a quantitative approach, analytical type, cross-sectional field design. A total of 37 pediatric patients admitted to the Pediatric Special Care and Intensive Care Unit (UCEP / PICU) of the Autonomous Institute of the University Hospital of Los Andes (IAHULA) who were under mechanical ventilation during the period from January to May 2018 were included in the sample. The nutritional status at the unit admission was evaluated by direct and indirect methods (anthropometry, dietetics, socioeconomic evaluation) and compared with the clinical evolution of the patients. Results: 56.8% were male patients, 45.9% infants, 51.4% were located in the IV socioeconomic level of the Graffar Scale, the place of admission was 70.3% UCEP and 29, 7% PICU, anthropometric diagnosis was distributed in 2.7% over the norm, 51.4% in the norm and 45.9% under the norm, statistically significant differences (p = 0.040) were found between the evolution of the clinical picture and anthropometric diagnosis. Conclusion: Nutritional status is a determining aspect so that patients under mechanical ventilation who enter the IAHULA PICU / UCEP evolve satisfactorily or negatively according to the basic clinical picture. Recommendations: Nutritionally assess and maintain constant nutritional epidemiological surveillance of all patients admitted to the PICU / UCEP of IAHULA.

    Factores asociados al estreñimiento en escolares, municipio libertador del estado Mérida Associated factors with constipation in schoolchildren, libertador municipality of the Merida state

    Objective: Evaluate the risk factors associated with constipation in schoolchildren in the Libertador municipality of the Merida state, Venezuela. Methodology: An observational cross-sectional investigation was carried out in 120 schoolchildren between 9 and 14 years of age. A nutritional evaluation was carried out and a questionnaire was applied to inquire about family history, eating habits, fiber intake, water, evacuation pattern, physical activity. It was divided into two groups: without constipation (GSE) and with constipation (EG). Results: There were no statistical differences between the groups regarding age, gender, institution and family history, the average age of the EG was 10.69 ± 1.29. In the EG, the normal nutritional state prevailed with 53.8 %, and 23.1 % above the norm. The daily evacuation rhythm of the NEG is (1.54 ± 0.79) vs. EG (1.15 ± 0,) (p = 0.01). Regarding the hours of physical activity, GNE (5.06 ± 6.12) vs. GE (4.27 ± 6.26). The fiber intake in the GNE was (9.05 ± 8.69) vs GE (6.42 ± 3,) (p = 0.02). Intake of glasses of water per day NEG (4.36 ± 2.51) vs EG (3.92 ± 1.98). Conclusion: the factors that seem to be associated were lack of habit to go to the bathroom, low fiber and water intake, and not practicing physical activity. It is important to involve not only the parents, the schoolchild and the pediatrician, but also the teacher, since school is where the child spends most of the day, being a key objective in the prevention and resolution of childhood constipation.

    Relación entre el índice de masa corporal y las cifras de tensión arterial en adolescentes

    Objetivos: Investigar la relación entre el índice de masa corporal (IMC) y las cifras de tensión arterial en adolescentes de la ciudad de Mérida, dada su asociación con el desarrollo de enfermedades crónicas no trasmisibles en el adulto. Métodos: Se \nestudiaron 385 adolescentes, 62,3% de sexo femenino y 37,7% de masculino, entre 12 y 15 años de edad, con promedio de 15,17±1,71 años. Se les tomaron las medidas antropométricas, para el cálculo del IMC, y la tensión arterial (TA) en posición sentada. Se consideraron normales el IMC y la TA comprendida entre los percentiles 10 y 90 para las curvas venezolanas, de acuerdo a edad y sexo; se consideró sobrepeso y obesidad sobre el pc 90 y Pre-hipertensión (Pre-HTA) e hipertensión arterial (HTA) sobre el pc 90. Resultados: El 75,3% de los adolescentes presentaron un IMC normal, el 11,2% un IMC bajo y el 13,6% sobrepeso y obesidad. El 1,1% presentò TA sistólica sobre el pc 90 y el 6,3% presentó TA diastólica sobre la norma. No se observó asociación del IMC y de la TA con el sexo ni el estrato socio-económico. Se encontró una alta y significativa asociación entre el sobrepeso/obesidad y la Pre-HTA/HTA (p=0,0001). El riesgo de un adolescente con IMC sobre el pc 90 de presentar Pre-HTA o HTA fue 9,76 veces mayor (Odss ratio) que el adolescente con IMC menor al pc 90 (IC 95%: 4,09-23,27; p=0,0001). Conclusión: Se comprobó una asociación estadística entre el IMC y los valores de TA sistólica y diastólica, por lo que adolescentes que tengan IMC altos deben ser seguidos y sometidos a algún tipo de intervención tendiente a disminuir la incidencia de enfermedades crónicas no transmisibles en la edad adulta.

    Soporte nutricional en pacientes pediátricos críticamente enfermos, Hospital Universitario de Los Andes, 2017 Nutritional support in critically ill pediatric patients. Autonomous Institute University Hospital of The Andes, 2017

    Los pacientes pediatricos criticos tienen mayor susceptibilidad a la desnutricion durante su estancia intrahospitalaria, en periodos cortos de ayuno o de insuficiente aporte nutricional que provocan deterioro nutricional y alteraciones en el crecimiento a corto y largo plazo. Objetivo: Analizar el soporte nutricional en pacientes pediatricos criticamente enfermos del Hospital Universitario de Los Andes Merida-Venezuela, 2017.Metodologia:enfoque cuantitativo, tipo analitico, diseno no experimental de corte transversal. Se estudiaron 53 pacientes ingresados en la Unidad de Cuidados Especiales (UCEP) y en la Unidad de Cuidados Criticos Pediatricos (UCIP). Resultados: 52,8% femenino, edad 3,91 ± 4,97 anos, 60,4% en el grupo de lactantes. 52,8% dias de hospitalizacion de 1 a 7 dias. Diagnostico medico de ingreso: 26,4% por enfermedades gastrointestinales. 43,4% bajo la norma en el diagnostico de crecimiento por talla. 94,3% de los pacientes tenian un soporte nutricional enteral; 86,8% no cumplio con el soporte nutricional indicado. En el soporte nutricional por medio de la prueba t de Student de muestras independientes se observa que en grasas se encontraron diferencias estadisticas (p=,003) y en carbohidratos (p= <,0,01). Se obtuvo deficiente adecuacion nutricional de requerimiento calorico total (47,2%), proteinas (32,1%), grasas (39,6%) y carbohidratos (56,6%). Conclusiones: Se determino un porcentaje moderado de pacientes que se encontraban bajo la norma en el diagnostico nutricional por peso, diagnostico nutricional por talla y de crecimiento por talla; los pacientes tenian en un alto porcentaje soporte nutricional enteral y un bajo cumplimiento del tratamiento nutricional indicado; se observo deficiente adecuacion nutricional. Critical pediatric patients are more susceptible to malnutrition during their hospital stay, during short periods of fasting or insufficient nutritional intake, causing nutritional deterioration and growth disturbances in the short and long term. Objective: To analyze the nutritional support in critically ill pediatric patients of the University Hospital of Los Andes Merida-Venezuela, 2017. Methodology: quantitative approach, analytical type, non-experimental cross-sectional design. 53 patients admitted to the Special Care Unit (UCEP) and the Pediatric Critical Care Unit (PICU) were studied. Results: 52.8% female, age 3.91 ± 4.97 years, 60.4% in the infant group. 52.8% days of hospitalization from 1 to 7 days. Admission medical diagnosis: 26.4% due to gastrointestinal diseases. 43.4% under the norm in the diagnosis of growth by height. 94.3% of the patients had enteral nutritional support; 86.8% did not comply with the indicated nutritional support. In the nutritional support by means of the Student´s test of independent samples, they were found statistical differences in facts (p =.003) and carbohydrates (p = <0.01). Poor nutritional adequacy of total caloric requirement (47.2%), protein (32.1%), fat (39.6%) and carbohydrates (56.6%) was obtained. Conclusions: A moderate percentage of patients who were under the norm in nutritional diagnosis by weight, nutritional diagnosis by height and growth by height was determined; the patients had a high percentage of enteral nutritional support and low compliance with the indicated nutritional treatment; poor nutritional adequacy was observed.
