Replacement of teeth lost by conservative procedures is part of the comprehensive treatment of clinical services of the dentist. However, these treatments in an older adult is one of the biggest challenges in restorative dentistry. This report describes a choice of conservative treatment that made in a patient of 91-year-old who attended a dental clinical consultation because of a total coronary fracture of the 42 dental unit. The patient has a systemic history and is carrier of a mitral valve prosthesis, so she consumed anticoagulant treatment for 30 years. Aims to preserve the root of 42, a tame the rest of the Crown of the tooth and a bridge of composite reinforced with post of fiberglass as main structural base were performed. With this clinical case it could be demonstrated that there are alternatives of urgent restorative treatments which can immediately restore a functional occlusion.
