Manejo clínico terapéutico de la osteonecrosis por uso de desvitalizante Devitex en paciente pediátrico. Reporte de caso.

Abstract

Introduction: Osteonecrosis is the death of bone tissue produced mostly by a decrease in blood flow. In the maxilla it can appear spontaneously after the application of a devitalizer when performing a pulp treatment without proper management. In pediatric patients, osteonecrosis is manifested by exposure of the bone at the affected site, inflammation of the adjacent soft tissues, halitosis, pain, and can produce mucous or cutaneous fistula.Objective: To describe the clinical therapeutic management of osteonecrosis due to the use of Devitex devitalizing agent in pediatric patients, through the application of a standardized treatment protocol. Case Report: 6-year-old female, arrives to consultation due to intense, acute pain and that doesn’t cease with medication. He was referred by a colleague who reported performing pulp therapy on tooth 85, using Devitex devitalizer. The clinical and radiographic examination revealed the presence of osteonecrosis around the treated tooth. For the rehabilitation of the patient, a standardized treatment protocol for osteonecrosis was applied, which consisted of: extraction of the affected tooth, bone and gingival remodeling, systemic medication with antibiotic therapy and controls for two years. Conclusions: The clinical therapeutic management of osteonecrosis due to the use of Devitex devitaliant in pediatric patients with the application of a standardized treatment protocol allows the control and total recovery of the patient in the medium and long term.
