Evaluación de la atención médica en la unidad de emergencias del hospital del niño "Dr. Ovidio Aliaga Uría"

Abstract
La calidad en salud es la provisión de servicios accesibles y equitativos ofrecidos con óptimo nivel y recurso profesional disponible para alcanzar satisfacción del usuario. La Organización Mundial de la Salud (OMS), estima que la tercera parte de la población mundial no tiene acceso a medicamentos esenciales. Material y métodos.- Estudio transversal, descriptivo realizado en Noviembre 2007. Se efectuaron 146 encuestas a los padres o responsables de pacientes del Seguro Universal Materno Infantil (SUMÍ) en Emergencias del Hospital del Niño" Dr. Ovidio Aliaga Uría" para evaluar la atención medica e indicadores de calidad OMS. Se excluyeron pacientes críticos y aquellos cuyos familiares negaron su colaboración. Resultados.- De 146 pacientes evaluados, 122 (83,5%) historias clínicas y 79 (57,2%) recetas fueron incompletas. Los indicadores de la OMS se cumplieron solo en el 85.5%. Discusión.- Encontramos que la consulta médica en su mayoría duró 15 minutos (77,6%). La proporción de prescripción de antibióticos fue 29.9% similar a informes internacionales (23,4% a 58,8%). El uso y disponibilidad de medicamentos esenciales fue de 97,4% mucho mayor a las publicaciones locales e internacionales. Conclusión.- Nosotros informamos un alto cumplimiento de los indicadores de calidad OMS en un hospital de tercer nivel y de enseñanza pediátrica. Las investigaciones sobre calidad en la atención médica son escasas por lo que incitamos a realizar estudios similares en otras áreas de las especialidades medicas.

According to the WHO estimated that the third world population doesn't have access to the essential medications. Methods.- Transverse and descriptive study carried out in November 2007, 146 surveys were realized to parents or caregivers of patients covered (Universal Infantile Maternal Sure) in an Emergency Unit of "Dr. Ovidio Aliga Uría" Children's Hospital Of La Paz Bolivia, to evaluate the quality of medical assistance according to the WHO quality indicators. Critical patients and those whose family refused to collaborate with the study were excluded. Results.- 146 evaluated patients, 122 (83,5%) clinical files and 79 (57,2%) prescriptions were incomplete. The WHO quality indicators they were completed in 85.5%. Discussion.- We found that the medical assistance lasted 15 minutes in 77,6%. The antibiotic prescription proportion was 29.9%, similar to other international reports of pediatric teaching centers (23,4% to 58,8%). The availability and use of essential medications was 97,4% bigger to local and international reports. Conclusion.- We report high-performance quality indicators WHO at a third level center of Pediatric Teaching Hospital. There are few researches of medical assistance quality in our country. Investigations in this field are still scarce, that's why we encouraged doing similar studies in other medical specialties.
