The Internal Resorption is a rare pulpar pathology. It generally occurs in permanent teeth, mainly in maxillary central incisors. This illness is associated to inflammatory processes of the pulpo-dentinal complex. It may be temporary or progressive. In this report, a case of internal inflammatory resorption is presented, with root canal calcification at the middle third and apex in the upper left lateral incisor in a 52 year-old patient. During the clinical examination she had a slight and spontaneous pain, and the radiographic examination showed a large internal resorption as well as an external resorption in the upper left central incisor. It was decided to apply conventional endodontic treatment: The root canal was obturated with calcium hydroxide cement (Sealapex®) in order to seal the defect. It was restored at the coronal level. There were clinical and radiographic examinations during 18 months, where it was seen that the internal resorption process stopped.
