Osteosarcoma is a malignant neoplasm derived from bone cells, its idiopathic etiology has been associated with genetic predisposition, trauma or radiation therapy. It appears in the jaws of young adults average age 33. Histologically characterized by atypical osteoblasts and abnormal osteoid bone formation. This case study presents the histopathological report of a lesion found in a male patient aged 63, diagnosed by incisional biopsy as grade II maxillary osteosarcoma with aggregated osteomyelitis. Radical surgery of the lesion was performed, followed by histopathological analisys that confirmed the initial osteosarcoma report with a higher grade of severity. It is suggested that this maxillary osteosarcoma, unusual for elderly people with history of squamous cell carcinoma, not treated with radiotherapy, had an idiopathic ethiology and probably associated with patients with genetic predisposition for malignant tumors.
