Acquired maxillary defects may have a pathological, traumatic or developmental origin and can alter vital functions such as breathing, chewing, phonation and swallowing. Being limited the surgical reconstruction possibilities limited, it is needed the use of prostheses to regain anatomy, function and esthetics. A clinical case of a child is presented were a right hemimaxillectomy was performed due as a consequence of a tumor. It was decided to build an immediate surgical obturator and another post surgical in thermoformed acetate No. 60, it was selected because of it’s light weight, thinness, smooth, low cost, ease of handling, great adaptation and retention to remaining oral structures. It can be easily replaced in case of change of the oral structures or wear. It is evident the importance of the interdisciplinary approach as well as some diagnostic errors, it’s consequences on how they were influential in the treatment. The obturators presented certain limitations, however, the advantage of ease for fabrication, cost, comfortable, functional, esthetics and versatility were predominant. The prosthetic purpose of the obturator is to separate the oral and nasal cavities in order to regain basic function as well as an acceptable esthetics. To obtain better quality of life for the patient are a must in the treatment plan.
