FRECUENCIA DE PRESENTACIÓN DE LESIONES VASCULARES EN EL TUBO DIGESTIVO ALTO I.G.B.J. (1979 - 2004)

Abstract
El presente trabajo, es una revisión retrospectiva de los estudios endoscópicos altos realizados en el I.G.B.J., en el período comprendido entre enero de 1979 y diciembre de 2005, incluyendo los diagnósticos de malformaciones o lesiones vasculares del tubo digestivo superior. Las malformaciones vasculares del tubo digestivo alto son lesiones asentadas sobre la mucosa intestinal, éstas pueden aparecer de forma hereditaria como la telangiectasia hereditaria familiar, de forma adquirida, como secuela de radioterapia, y de forma esporádica generalmente en pacientes de edad avanzada como en las malformaciones vasculares primarias (ectasias vasculares, angiodisplasia, enfermedad de Dieulafoy). Son causa frecuente de hemorragia digestiva alta y baja en mayores de 60 años. Detectadas en el 5-8% de las gastroscopías realizadas, siendo causan del 2,1 al 8% de las hemorragias digestivas altas. Nuestro objetivo es determinar la frecuencia de presentación, su relación con factores como edad, sexo, morbimortalidad, tratamiento y evolución. Se revisó un total de 40.714 estudios endoscópicos de los cuales 31.633 (77.7%) corresponden a endoscopias altas. Del total de estudios, se encontró 99 pacientes con diagnóstico de lesión o malformación vascular lo que corresponde al 0.31%. Las malformación vascular es una entidad patológica diagnosticada con mayor frecuencia los últimos años, con una incidencia en relación a la hemorragia digestiva alta de 2,43%. Las malformaciones vasculares en el tubo digestivo superior son más frecuentes en el estómago (76,76%) luego esófago y duodeno. La lesión vascular de Dieulafoy es la malformación más frecuente (55,55%) y su relación con la H.D.A. es del 100%. El tratamiento está basado prácticamente en el manejo de la H.D.A. utilizando la endoscopía terapéutica como método seguro y eficaz.

They are frequent cause of high and low digestive hemorrhage in greater of 60 years. Detected in the 5-8% of gastroscopías made, being cause from the 2.1 to 8% of high the digestive hemorrhages. Our objective is to determine the presentation frequency, its relation with factors like age, sex, morbimortalidad, treatment and evolution. A total of 40,714 endoscópicos studies was reviewed of which 31,633 (77.7%) correspond to high endoscopias. Of the total of studies, one was 99 patients with diagnosis of injury or vascular malformation what corresponds to the 0.31%. The vascular malformation is a pathological organization diagnosed most frequently the last years, with an incidence in relation to high the digestive hemorrhage of 2,43%. The malformations vasculares in the superior alimentary canal are more frequent in the stomach (76,76%) followed of esophagus and duodeno. The vascular injury of Dieulafoy is the most frequent malformation (55,55%) and its relation with the H.D.A. it is of the 100%. The treatment is based practically on the handling of the H.D.A. using endoscopía therapeutic like safe and effective method.
