Abstract
Aerococcus urinae como causa de infección urinaria en ancianos con factores de riesgo es una presentación cada vez más frecuente como patógeno emergente que podría llevar a un desenlace catastrófico. Aerococcus spp. pasaba desapercibido en los urocultivos al ser confundido con un contaminante, no fue hasta la llegada del MALDI-TOF que se empezó a identificar con claridad, dando a conocer que su baja prevalencia se encuentra infraestimada. La sintomatología de infección urinaria que presenta es indistinguible del síndrome miccional habitual y el peligro que conlleva es el posible desarrollo de bacteriemia y/o endocarditis infecciosa, por lo que la antibioticoterapia correcta basada en el antibiograma es fundamental. Se presenta el caso de infección urinaria por Aerococcus urinae en una paciente añosa con deterioro cognitivo, infecciones urinarias de repetición, incontinencia urinaria e inmovilización. Las características en el urocultivo y, sobre todo, la espectrometría de masas fueron esenciales para el diagnóstico.
Aerococcus urinae as a cause of urinary tract infection in elderly patients with risk factors is an increasingly common presentation as an emerging pathogen that could lead to a catastrophic outcome. Aerococcus spp. went unnoticed in urine cultures because it was mistaken for a contaminant. It was not until the arrival of MALDI- TOF that it began to be clearly identified, revealing that its low prevalence is underestimated. The symptoms of urinary tract infection that it presents are indistinguishable from the usual micturition syndrome and the danger it entails is the possible development of bacteremia and/or infective endocarditis, so correct antibiotic therapy based on antibiogram is essential. We present the case of urinary tract infection by Aerococcus urinae in an elderly patient with cognitive impairment, recurrent urinary tract infections, urinary incontinence and immobilization. The characteristics of the urine culture and especially the mass spectrometry were essential for the diagnosis.
