REPORTE DE CASO: MANEJO QUIRÚRGICO: ANEURISMA BI-ILIACO

Abstract
Los aneurismas de la arteria iliaca (AAI) son extremadamente raros, y las descripciones de que se presenten en ambas iliacas es más raro aún. Los AAI son más frecuentes en el sexo masculino, con una relación 7:1. Se estima que su incidencia en Bolivia es del 0,03%. Se presenta el caso de un paciente masculino de 75 años de edad, cardiópata, portador de marcapasos y chagasico. Acude por cuadro clínico de mas o menos 3 semanas de evolución caracterizado por dolor lumbar posterior a esfuerzos. Laboratorios normales. Se le realiza una tomografía con contraste que reporta aneurisma iliaco bilateral. Por los diámetros mayor a 3cm y clínica compresiva, se decide la intervención quirúrgica abierta. Se realiza una derivación aorto-bifemoral, que evoluciona favorablemente.

Their incidence in Bolivia is estimated to be 0.03%. We present the case of a 75-year-old male patient with heart disease, a pacemaker, and Chagas disease. He presented with a clinical picture of approximately 3 weeks of post-exertional lower back pain. Laboratory results were normal. A CT scan with contrast revealed a bilateral iliac aneurysm. Due to diameters greater than 3 cm and compressive symptoms, open surgery was decided upon. An aorto- bifemoral bypass was performed, which progressed favorably.
