Abstract
Se presenta un caso del primer ByPass cerebro vascular realizado en Bolivia por un aneurisma gigante de arteria carótida interna derecha, en segmento cavernoso, tratado mediante bypass de alto flujo de arteria carótida externa a arteria cerebral media derecha, con injerto de arteria radial y posterior oclusión y exclusión del aneurisma. Se describen los pasos de la cirugía y se destacan las dificultades de la técnica y los beneficios de la cirugía. Se discuten los elementos a considerar en la toma de decisiones para la indicación y realización del bypass en el tratamiento de los aneurismas gigantes de la ACI.
A case report for the first Cerebral ByPass in Bolivia of a giant aneurysm arising from the right internal carotid artery, cavernous segment, treated with a high flow radial artery graft from the external carotid artery to the right middle cerebral artery M2-M3, and subsequent occlusion and exclusion of the giant aneurysm. The steps of the surgery are described and the difficulties of the technique and the benefits of the surgery are highlighted. The elements to be taken in consideration in the indication and realization of the bypass surgery in the treatment of the ICA giant aneurysm are discussed.
